Refer a veteran
Someone you knowis carrying it alone.
If there is a veteran in your family, your unit, your congregation, or your caseload who has gone quiet, tell us about them. Family members, friends, caregivers, service providers, and partners can all make a referral — no referral form or diagnosis required.
What happens next
You send their name
A name and how to reach them is enough. You do not need their paperwork, a discharge status, or a diagnosis.
We reach out the way you asked
Veteran first, or you first — whichever you choose. One phone call or email, and no pressure if the timing is wrong.
They choose what comes next
A conversation, a weekly circle, or a referral onward. Programs are free, and nobody is enrolled without their own say.
Make the referral
Tell us aboutthem.
Fill in what you know and leave the rest blank. Our team reads every referral and follows up within two business days.
If the veteran is in crisis right now, do not wait on this form: dial 988, then press 1 for the Veterans Crisis Line, or text 838255.
